Cheat mode:
Press Up(2), Down(2), A, B, A, B at the title screen. Then hold A and press Start. Press Up or Down to select a new starting level, with 80 lives, and 99 miracle kicks will now be available.

Continue game:
Hold A + B and press Start when the title screen reappears after the game over screen.
